Below are some frequently asked questions.

 

 Need assistance to apply for the NDIS?

Carers Queensland is a partner of the National Disability Insurance Scheme (NDIS), and help to deliver the NDIS in Queensland.

They are the largest local area coordination (LAC) in Queensland.  Part of their role is to help people who may be eligible for the NDIS to learn more about applying for the scheme.

Yes, Support Coordination and / or Psychosocial Recovery Coaching MUST be specifically funded in your approved NDIS plan to access our services.

Where deemed reasonable and necessary by the NDIS, support coordination funding will be included in the Capacity Building budget.  This is a fixed amount for a Support Coordinator and/or Psychosocial Recovery Coach to help you use your plan.  

If your plan describes (or ‘states’) the level of support coordination funded, you can only purchase this level of support coordination.  If your plan does not describe the level of support coordination funded, you can choose to purchase the level of support coordination that suits your needs.

Please note Chryslais Collective only deliver Level 2: Coordination of Supports and Psychosocial Recovery Coaching.

 

Do we have capacity for new Support Coordination / Psychosocial Recovery Coaching participants?

 

IMPORTANT: Presently we have paused all referrals for Support Coordination and Psychosocial Recovery Coaching due to reshuffling of caseloads.

 

Additional Information:

Please note we do require a minimum of 48 hours per year of funding in order to efficiently support our participants and meet our registered provider obligations.

Support Coordinators / Psychosocial Recovery Coaches are match based on capacity, locality, experience and compatibility that best suits the individual participant.

Do we have capacity for new Peer / Disability Support participants?

We presently have limited capacity for clients requiring Peer / Disability Support.

We will be looking to expand recruitment in 2026.

 

What is the difference between a Support Coordinator, Psychosocial Recovery Coach and Peer or Disability Support Worker?

What is our current service area?

Due to the limitations on provider travel charging in the NDIS price guide, we only service the Sunshine Coast area and surrounds to ensure our staff are adequately reimbursed for their travel time and expenses for use of their own vehicles.

 

What is our referral process?

Head to our ‘Contact Us’ webpage and download, complete and return the Referral Form together with any requested documentation prompted throughout completion. The form is available online in both PDF and Word versions.  Alternatively, we can email or post the Referral Form out to you.

All referrals are subject to review and acceptance by our Directors.  The risk assessment included in the Referral Form assists our Directors to ensure a participant is appropriately matched and safety measures are put in place to mitigate any identified risks.  Once your Referral Form has been reviewed, we will be in touch regarding next steps.

 

Can you request a meet and greet?

Absolutely!

A meet and greet is a great way to overcome any anxiety you may have about working with a new provider.  This meeting is to also see if both parties are well matched and willing to work with each other.

Prior to this meeting, we will provide you with a link to our organisation’s webpage which details our company’s services, vision and team member’s experience and qualifications.  We will also still require a Referral Form to be completed and returned to us including the brief risk assessment and requested documentation.

On the day, we will bring a service agreement.  You are by no means obligated to sign this but it streamlines our processes if you wish to proceed.  Please note we are unable to bill for this time or travel expense unless a service agreement is signed on the same day.  Meet and greets are therefore limited to 30 mins.

As a small organisation, we must manage this expense accordingly as a cost of business.  We appreciate your understanding.

Do we have an office?

No, we work remotely to keep our business overheads low.

All face-to-face meetings with participants, informal supports and / or stakeholders are conducted either at the participant’s home or in the community.

All home visits are subject to a Home Visit Risk Assessment.

 

What are our operating hours?

Our operating hours are Monday to Thursday 8:00am to 4:00pm, and Friday 8:00am to 12:00pm excluding Public Holidays and 2 weeks over the Christmas / New Year period.
